Your First Visit

In order to make your first visit to Lake Stevens Vision Clinic, Inc. as easy and smooth as possible, we have provided on this web-site a way to download and fill out some of the forms we will need in order to collect some important information to process your first examination. By filling out these forms before you arrive at our clinic, we are able to provide a much more timely transition from entry to exit, without uncomfortable delays.

Some items to bring with you on your first visit to Lake Stevens Vision Clinic, Inc., and things to remember include:

  • Bring your insurance card with you so that we can scan a copy into your records.
  • Bring glasses from your past examinations with you so that we can have an idea of how your eyes have changed and how well you have adapted to your glasses.
  • Bring copies of your current spectacle and contact lens prescriptions if available.
  • If you are a contact lens wearer, always wear your contact lenses into the exam room with at least three hours of wear on the day of the exam.
  • Bring filled in copies of our Eye Health History and General Health History forms.
  • Bring in a signed copy of our Lifetime Authorization Form.
